If you're a new 1st Source customer please fill out the BankOnCampus application completely. Be sure to include co-applicant information, which is required for the Resource Plus® card. Upon completion of the application, all the information you have entered will be displayed on a printable form. You must PRINT THIS FORM, SIGN IT, and MAIL IT to the address indicated.

Within one business day, you'll receive an e-mail from a BankOnCampus representative informing you that your application is being processed. You will be reminded at that time to send in your printed application. Your account cannot be finalized until the printed application is received.
